QIcon: Prefer high-quality images of Windows .ico files.

Refactor the code QPixmapIconEngine::addFile() using a convenience
class for reading all images.
Special-case .ico-files: Read images into a list and replace by
higher-quality ones.

Task-number: QTBUG-39287
