Friedemann Kleint 7a8883c434 Fix naming of plugins for QPA plugins and use new plugin system. 
- Fix naming "com.nokia" -> "org.qt-project" in platform
  integration & platform theme.
- Adapt Windows, XCB, Cocoa, Minimal.
