Added QDuplicateTracker to keep track of used tags, rather than looping
through QTestTablePrivate::dataList for every added tag.
Removed method `hasRow`, instead calling `QDuplicateTracker::hasSeen`
directly in `newData`.

Tero Heikkinen caught tst_QQuaternion() using a duplicated test data
tag and was surprised that testlib let it get away with that. That
seems like a reasonable thing to discourage. While I'm at it,
duplicate columns should be discouraged.
[ChangeLog][QtTest] Duplicate data tags are now warned about. Every
call to QTest::newRow() or QTest::addRow() should result in a distinct
data tag. If they do not, a warning is produced. Likewise, duplicate
column names are forbidden; each call to QTest::addColumn() should use
a distinct name.

Since 7a42c8c15c QTestTablePrivate no
longer cleans up the memory it holds in dataList. This will make tools
such as valgrind/address-sanitizer generate a lot of noise about
directly or indirectly leaked memory.

This speeds up tests/auto/corelib/tools/qchar which spends a lot
of time in QTestTablePrivate::elementAt(), QTestTablePrivate::dataAt()
from 6s to 500ms on an average Linux machine.

Qt copyrights are now in The Qt Company, so we could update the source
code headers accordingly. In the same go we should also fix the links to
point to qt.io.
Outdated header.LGPL removed (use header.LGPL21 instead)
Old header.LGPL3 renamed to header.LGPL3-COMM to match actual licensing
combination. New header.LGPL-COMM taken in the use file which were
using old header.LGPL3 (src/plugins/platforms/android/extract.cpp)
Added new header.LGPL3 containing Commercial + LGPLv3 + GPLv2 license
combination

As in the past, to avoid rewriting various autotests that contain
line-number information, an extra blank line has been inserted at the
end of the license text to ensure that this commit does not change the
total number of lines in the license header.

The private class had two append() methods, one appending a column and
one appending a row. Use more meaningful names instead of overloading
orthogonal operations.
