QModelIndex is not safe to be used to store an index as it is designed
to be discarded right after use as the index information can change.
Therefore a QPersistentModelIndex should be used instead to store the
index. Subsequently the m_index does not need to be updated whenever
the model changes anymore as this is already done for us.

cases where we can avoid/reduce allocations or/and reduce
text size, e.g.:

The new modular configuration system requires one global
header per module, that is included by all other files in
this module.
That header will later on #include the configuration file
for Qt Widgets. For now it defines the Q_WIDGETS_EXPORT
macro for this library.

The printf-style version of QDebug expands to a lot less code than the
std::ostream-style version. Of course, you pay in type safety (but
compilers warn about it these days), you cannot stream complex Qt
types and streaming QStrings is awkward, but in many cases you
actually improve on readability.
But the main reason is that something that's not supposed to be
executed under normal operation has no business bloating executable
code size.
This is not an attempt at converting all qWarnings() to printf-style,
only the low-hanging fruit.
In this second part, replace
qWarning() << "" << non-QString
with
qWarning("..%.", non-QString).
QString (and QUrl etc) have special escaping handling when streamed
into QDebug, so leave those alone. They also seem to expand to less
code than the qPrintable() alternative, so there's no reason to
replace them.
Saves 2KiB, 3.4KiB, ~750b and ~450b in text size in QtCore, Gui,
Network and Widgets, resp., on optimized GCC 5.3 AMD64 builds.

Currently, a tab's text is used as its accessibleName.
When a tab's text is empty, there is no API to set
the tab's accessibleName. The two APIs are added to
set and return the accessibleName property of a tab.

Resetting focus_window and other internal QGuiApplication variables before
calling setVisible(false) and destroying the platform window means that the
platform window can't reason about whether or not it was the focus window
unless it can resolve that using native APIs. We should let the platform
window take care of resetting the focus window and related states, and
only execute our fallback logic if the plugin doesn't do the right
thing.
We also use QPA to update the state instead of modifying the internal
QGuiApplication variables directly, so that events and signals are
emitted as a result of the reset.
The QLineEdit test gets two added calls to processEvents(), since
assuming that activateWindow() is synchronous is not correct, and
would result in the QMenu resetting the focus window to 0 on destroy.

The QMap<QByteArray, QString> was only used to hold key/value pairs
until they were serialized into a QString at the end of the function.
Serialize into QString directly instead, dropping the temporary QMap.
The problem is that we'd like to keep the line formatting central
while at the same time harnessing the power of QStringBuilder.
This is possible with a templated function with an input/output
parameter:
format_attr(QString &result, const char *key, T &&value)
format_attr(result, text-foo, str + str2 + str3);
Or with return type deduction:
formatted(const char *key, T &&value) -> decltype((expr)) { return expr; }
result += formatted(text-foo, str + str2 + str3);
I don't like out parameters, and we can't rely on auto return
type deduction, yet, so I opted for a miniature expression
template solution that can only match the expression
attr[key] = value;
where 'key' is a const char* and 'value' can be anything that
QStringBuilder supports. This allows to keep the syntax of a map
while at the same time serializing to QString immediately.
The only behavioral difference to the old code is that the
attributes are no longer sorted, but order doesn't matter.
Saves more than 10KiB in text size on optimized GCC 4.9 Linux
AMD64 builds.

This needs to be handled a bit carefully, because Qt containers
will detach upon being iteratoed over using range-for.
In the cases of this patch, that cannot happen, because all
containers are marked as const (either by this patch or before).
Separate patches will deal with other situations.
Apart from being more readable, range-for loops are also the
most efficient for loop.
This patch shaves almost 2K of text size off an optimized Linux
AMD64 GCC 4.9 build.

If, after checking a condition, we issue a qWarning(),
by definition that check is unlikely to be true.
Tell the compiler so it can move the error handling
code out of the normal code path to increase the
effective icache size.
This change contains the changes to the accessible/,
effects/, kernel/, styles/ and itemviews/ subdirs.
Moved conditional code around where possible so that
we could always use Q_UNLIKELY, instead of having to
revert to Q_LIKELY here and there.
In QWidgetPrivate::setWindowModified_helper(), as a
drive-by, I swapped the evaluation order of an
&&-expression (newly wrapped in Q_UNLIKELY) to be
more readable and more efficient (cheaper check
first) at the same time.
In qDraw* (qdrawutil.cpp), simplified boolean
expressions (sometimes by skipping re-checking
conditions already checked in a previous guard clause).

Current code retrieved the character format at position where the cursor
currently was, irrespective of the position actually passed to the
characterRect function. This is now fixed and we retrieve the character
format for the QTextFragment containing the specified offset.
This fixes display of visual bounds around text by screen reader in some
cases. E.g. on OS X, when searching text using VoiceOver, VoiceOver
first queries the visual bounds of the found text (and thus also
characterRect), and only then it moves cursor position to the found
text. If before the change of position the cursor was on some text with
different metrics (i.e. a bigger font), the visual bounds for the found
text reflected those metrics, not the metrics of the actual found text
for which the bounds were drawn. This resulted in smaller/bigger
rectangles around the found text than was actually correct.

QTextFormat::font{Weight,PointSize}() etc. return the font trait explicitly
set on the QTextFormat, while QTextFormat::font() returns font which also
uses the QTextDocument::defaultFont() as a default for any font traits not
explicitly set on the QTextFormat. Accessibility support for text attributes
used the former, which was wrong; this commit fixes it to use the latter.
Also includes tests to verify the fix.

On both iOS and Android it is very confusing to be able to move the focus to both, the line edit and the outer frame that is the spin box.
For Linux this fixes an issue that orca would not read the value correctly after pressing the up/down buttons.

The selectable/selected states refer to items in a list and similar,
do not interpret them as text selection states.
Without this change NVDA for example announces text edits as selected
which makes no sense and which it doesn't do for native text items.

This happens for example when running several tests.
Widgets in destructor should be treated as invalid
since their window pointer and other properties are
no longer valid.
When deleting a window containing only a table view
there would be a table model reset update comming from
the window being destroyed.

Add more testing.
This uncovers that currently the QDesktopScreenWidget
shows up as child of the app. Fixed by not creating
QAccessibleInterfaces for QDesktopScreenWidget.
Also don't crash in indexOfChild when called with 0.

While I do like the idea, it currently relies on every
QWidget having QAccessibleWidget as a11y representation.
This crashes for example when using the itemviews and
asking them for relations.

This operation should be a no-op anyway, since at this point in time,
the fromAscii and toAscii functions simply call their fromLatin1 and
toLatin1 counterparts.

Changed qt_accStripAmp implementation to handle texts which contains
pairs of ampersands representing a single ampersand.
In order to do that, a new static function called qt_accAmpIndex was
created.
This function is based on the code of qt_accHotKey, which was changed
to use qt_accAmpIndex.

QWidget::isActiveWindow() was being checked in QAccessibleWidget::state
to determine if a widget is focusable. As a result, focusable widgets
were reported to be not focusable when the window was not active.

As in the past, to avoid rewriting various autotests that contain
line-number information, an extra blank line has been inserted at the
end of the license text to ensure that this commit does not change the
total number of lines in the license header.

navigate() to Self does not make any sense (its basically a clone).
It seems that its not that useful to return Self from relationTo(),
since it was only one place where relationTo() was called where it
checked for the Self flag. This was in the windows bridge, and we
could easily work around that.
If it really turns out that Self is useful, we can always add that
enum value back later.

The state should contain useful and clear information.
Some of the old enum/bitfield members were not really clear.
Make them follow Qt terminology and shift the burden of interpreting
them to the bridge.
Apart from the previous commit changing from enum to bitfield, these
flags have changed names:
unavailable -> disabled
mixed -> checkStateMixed
protected -> passwordEdit (in the last commit)
floating is completely removed, even MSAA documentation states it is unsupported.
Some new states have been added.

We would like to add more flags that will be over the 32 bit boundary.
On Windows enums don't seem to digest values >32 bit.
This patch changes the state flags to be a bit field instead.
The windows part of the patch was written by Jan-Arve Sæther.

These are deprecated in favor of
QAccessibleInterface::child() and QAccessibleInterface::parent()
QAccessible::Sibling can be done with a combination of those two.
This is handled by the bridges, if required.

childAt used to return an integer.
Return an interface instead.
Not requiring a direct child to be returned allows optimizing
by bypassing iterating through the hierarchy of accessibles.
For QtQuick this is the only sensible way of implementing this.
The bridges are still responsible for finding the top-most element.
The default implementation in QAccessibleObject is sufficient
to return direct children. The implementation in
QAccessibleApplication is therfore no longer needed.

This makes the accessibility apis much
simpler and less error prone.
Disable the itemviews implementation that is in complex widgets.
The itemviews will use the new code from itemviews.h/cpp everywhere now.
QToolBox was broken before, now at least it simply exposes all its children.
The children are the buttons (tabs of the toolbox) and their contents.

Stop the mis-use of navigate to find the parent.
In order to make navigation straight forward
parent and child functions are now part of
QAccessibleInterface.
This allows navigating the hierarchy of
accessible objects without the 1-based indexes in the
navigate function which lead to confusion.
Eventually the support for Ancestor in navigate can be completely removed
and forwarded in the windows bridge if needed.
In addition default parameters for virtual children.
This will make the transition smooth since it allows to remove the integer
already.

Add a default implementation for RelationTo.
Usually this should be empty and it is pointless to return
the parent-child releations in this function.
Instead we will have proper parent-child functions and also
remove this functionallity from navigate.
Having default implementations for these removes the
necessity to reimplement them to return Unrelated in
many places.
